Education Health and Care Plans and Appletree’s Outcome Based Care Planning

From September 2014 education, health and care plans (EHC) plans replaced the statement of educational needs and learning disability assessment.

How does Appletree play a role in this?

As an organisation, this means we are now working more closed with multi – agency professionals, the child and family to achieve common outcomes as set out in the EHC plan.

Our current care plans and assessments have been developed in line with the outcomes in order to be part of a consistent approach in care and support as well as education for the child and young person.

 

Supporting Individual Progress

From January 2015 we will be implementing a new outcome based care plan which will have more emphasis on and reflect the individuality and progress of the children and young people with work with. 

Appletree’s new care plans are drawn up in collaboration with service users, their families and other relevant professionals to target specific outcomes. This means that, as they provide support, our support workers will which will help children and young people to make progress towards these outcomes.
